By submitting this entry, I hereby, for myself, my heirs, my executors, and administrators, waive, release and discharge all and any liability, claims, demands and rights for damages of any kind or nature that I may at any time, against CHAMP Cancer Hope & Awareness Month for Pediatrics, USATF, collaborating Nonprofits, and/or the sponsors of the CHAMP Event (including respective affiliates, volunteers, officers, directors of the sponsors), Event Race Timing Systems, Breakfast volunteers, coordinating groups and any individuals associated with the event, their representatives, and will indemnify and hold each harmless for, from and against any and all injuries, illness or damages that I suffer or cause during or in connection with the CHAMP Event, whether foreseen or unforeseen, from negligence or otherwise. I hereby consent to medical treatment in the event of injury, accident or illness during said event. I hereby grant full permission to any and all the foregoing to use my likeness name in any print or reported media, video, telecast related to or advertising the event.

I attest that I will not attend the event if I am showing symptoms of Covid-19, or have been in close proximity to a confirmed case within days of the event.

In the event of developing a fever or becoming unwell at the event, I will report this to CHAMP organizers, leave immediately, and follow up with medical professionals as necessary.
